Semaglutide is a synthetic peptide analog of human glucagon-like peptide-1, a gut hormone released after meals. Its backbone retains the GLP-1 sequence but incorporates two substitutions that slow enzymatic breakdown by dipeptidyl peptidase-4. A short polyethylene glycol linker and a C18 fatty diacid are attached to the peptide chain, allowing the molecule to bind serum albumin and remain in circulation far longer than the native hormone. The result is a circulating half-life measured in days rather than the minutes typical of endogenous GLP-1.
Receptor activation occurs at GLP-1 receptors distributed across pancreatic islets, the hypothalamus, and the gastrointestinal tract. Binding triggers G protein signaling that raises cyclic AMP and enhances glucose-dependent insulin release. Because the effect depends on prevailing glucose levels, insulin secretion does not rise when blood sugar is already low. Signaling in the brain and gut also influences appetite and gastric emptying, which is why the compound appears in both metabolic and weight-related research literature.
The compound binds the GLP-1 receptor on pancreatic beta cells and other tissues, activating a G-protein signaling cascade that raises intracellular cyclic AMP. This action increases glucose-dependent insulin secretion when blood glucose is elevated, while binding also slows gastric emptying and reduces glucagon release. In the central nervous system, receptor activation in the hypothalamus and brainstem contributes to reduced appetite. The fatty acid chain binds albumin, which protects the peptide from renal filtration and enzymatic degradation. This albumin binding is central to its extended circulation time.
Native GLP-1 is degraded rapidly by dipeptidyl peptidase-4. Semaglutide resists this cleavage because alanine at position 8 is replaced by alpha-aminoisobutyric acid. A second substitution at position 34 introduces arginine, which further stabilizes the peptide. The most distinctive modification is a spacer and C18 fatty diacid attached at lysine 26, enabling strong albumin affinity. These three changes together produce a half-life measured in days rather than minutes, and the same structural logic underlies other long-acting analogs in this class.

Clinical studies of semaglutide generally measure glycated hemoglobin, fasting plasma glucose, body weight, and composite cardiovascular endpoints. The SUSTAIN program enrolled adults with type 2 diabetes, while the STEP program focused on obesity without diabetes. Administration follows a stepwise escalation schedule designed to limit gastrointestinal effects during the first weeks. Reported outcomes include mean percentage weight change, the proportion of participants reaching defined weight-loss thresholds, and rates of nausea, vomiting, and diarrhea. Long-term data on durability after treatment stops are still limited and remain a topic of ongoing research.
Semaglutide is a synthetic peptide analog of human glucagon-like peptide-1, developed by Novo Nordisk and first approved in 2017 for type 2 diabetes. It belongs to the incretin mimetic class, a group of agents that reproduce the glucose-dependent actions of endogenous GLP-1. The molecule was engineered to resist degradation by dipeptidyl peptidase-4 and to bind serum albumin, extending its half-life from minutes to roughly one week. Approval for chronic weight management followed in 2021, based on large cardiovascular and obesity outcome trials.
GLP-1 receptors are expressed on pancreatic beta cells, in the gut, and in several brain regions. Receptor activation raises cyclic AMP, enhances glucose-dependent insulin secretion, and suppresses glucagon release when blood glucose is high. Effects on gastric emptying and on hypothalamic appetite circuits reduce energy intake. Because insulin release remains glucose-dependent, the risk of hypoglycemia is low when the drug is used alone. The precise contribution of each pathway to body weight change in humans remains an area of active investigation.

=== Judaism === Israeli prime minister Benjamin Netanyahu referenced the Torah and compared Iran to the ancient biblical enemy of the Amalekites, stating "We read in this week's Torah portion, 'Remember what Amalek did to you.' We remember—and we act." The Council on American–Islamic Relations condemned Netanyahu for "once again using the biblical story of Amalek—which claims that God commanded the Israelites to murder every man, woman, child and animal in a pagan nation that attacked them—to justify Israel's mass murder of civilians in Iran, just as it did in Gaza."

=== Reconstitution in exile === In January 1921, Russian émigrés meeting in Warsaw re-established the organization under the name People's Union for the Defense of the Motherland and Freedom. The reconstituted organization sought to continue anti-Bolshevik activity from abroad and to rebuild an underground network inside Soviet territory. Between 1921 and 1923, the People's Union engaged in underground and subversive activities directed against the Bolshevik regime. Volunteers and operatives were sent into the Soviet Union in an effort to organize armed detachments, establish clandestine cells, recruit supporters, and attempt to provoke a broader anti-Bolshevik uprising.
Even before the partitions from the late 18th century, the Russian Empire had already acquired some territories of the Polish–Lithuanian Commonwealth (a real union of Kingdom of Poland with the Grand Duchy of Lithuania). The first Russian partition took place in the late 17th century when the forced Treaty of Andrusovo signed in 1667 granted Russia the Commonwealth's territory in the Eastern Ukraine. Under the Third Partition of Poland Russia acquired Courland, all Lithuanian territory east of the Nieman River, and the remaining parts of Volhynian Ukraine.
In return, the host provides the symbiont with chemicals required for chemosynthesis, such as carbon, sulfide, and oxygen. In the early stages of studying life at hydrothermal vents, there were differing theories regarding the mechanisms by which multicellular organisms were able to acquire nutrients from these environments, and how they were able to survive in such extreme conditions. In 1977, it was hypothesized that the chemoautotrophic bacteria at hydrothermal vents might be responsible for contributing to the diet of suspension-feeding bivalves. Finally, in 1981, it was understood that giant tubeworm nutrition acquisition occurred as a result of chemoautotrophic bacterial endosymbionts. As scientists continued to study life at hydrothermal vents, it was understood that symbiotic relationships between chemoautotrophs and macrofauna invertebrate species was ubiquitous. For instance, in 1983, clam gill tissue was confirmed to contain bacterial endosymbionts; in 1984 vent bathymodiolid mussels and vesicomyid clams were also found to carry endosymbionts. However, the mechanisms by which organisms acquire their symbionts differ, as do the metabolic relationships. For instance, tubeworms have no mouth and no gut, but they do have a "trophosome", which is where they deal with nutrition and where their endosymbionts are found. They also have a bright red plume, which they use to uptake compounds such as O, H2S, and CO2, which feed the endosymbionts in their trophosome.

A probable diagnosis is based on the history of the illness and cognitive testing, with medical imaging and blood tests to rule out other possible causes. Initial symptoms are often mistaken for normal brain aging. Examination of brain tissue is the only way to definitively diagnose Alzheimer's, but neuroimaging techniques and fluid biomarkers have made it possible to diagnose probable Alzheimer's disease in vivo. No known treatments can stop or reverse its progression, though some may temporarily improve symptoms. A healthy diet, physical activity, and social engagement are generally beneficial in aging, and may help in reducing the risk of cognitive decline and Alzheimer's. Affected people become increasingly reliant on others for assistance, often placing a burden on caregivers. The pressures can include social, psychological, physical, and economic elements. Exercise programs may be beneficial with respect to activities of daily living and can potentially improve outcomes. Behavioral problems or psychosis due to dementia are sometimes treated with antipsychotics, but this has an increased risk of early death. As of 2020, there were approximately 50 million people worldwide with Alzheimer's disease. It most often begins in people over 65 years of age, although up to 10% of cases are early-onset impacting those in their 30s to mid-60s. It affects about 6% of people 65 years and older, and women more often than men. The disease is named after German psychiatrist and pathologist Alois Alzheimer, who first described it in 1906.
Cambridge selected cleaver blades for the first time in the history of the race, following the successful use of the oars in the 1992 Summer Olympics in Barcelona. The surface area of the cleaver was approximately 20% larger than the conventional macon blades. Oxford practised with cleaver blades in some of their outings leading up to the race, and were prepared to use them should the weather conditions be suitable, but on the day itself they opted to remain with the macon blades. The umpire for the race was the Canadian Olympic gold medallist and former Oxford Blue Mark Evans who had rowed in the 1983 and 1984 races. He caused controversy by instigating a new starting method whereby he would hold the crews for up to ten seconds between the conventional "set" and "go" commands. Evans had umpired the 1991 race in which he had also used his own starting method in preference to the traditional Amateur Rowing Association instructions. In response to any potential delay at the start during which time the boats will be dragged along with the tide, one of the stake-boat men, Bob Hastings, responsible for holding the boats until the "go" command is given stated: "If the boats start to drag I will let go, before I am dragged out of the stake-boat". Both Alan Inns, former coach and advisor to Cambridge, and Steve Royle, Oxford's director of rowing, expressed concerns over Evans' methodology.
=== Hematologic involvement === Hematologic abnormalities are prevalent in MCTD. Mild lymphadenopathy affects 25–50% of patients, and it is frequently an early symptom of the disease. This usually subsides over time; however, it may reappear during flares. Between 50% and 75% of people with MCTD will experience anemia, lymphopenia, or leukopenia. Anemia of chronic disease is the most common type of anemia seen in MCTD. Thrombocytopenia can develop in MCTD but is less common than leukopenia or anemia.
Post-mortem toxicology testing detected multiple substances for almost all those who died, with fentanyl and fentanyl analogs being the most frequently identified co-occurring substances. Overdoses of kratom are managed similarly to other opioid overdoses, and naloxone can be considered to treat an overdose that results in a reduced impulse to breathe, despite mixed results for its utility, based on animal models. From October 2017 to February 2018 in the United States, 28 people in 20 different states were infected with salmonella, an outbreak linked to the consumption of contaminated pills, powder, tea, or unidentified sources of kratom. An analytical method using whole genome sequencing applied to samples from the infected users indicated that the salmonella outbreak likely had a common kratom source.
== Structure == SrtB overall structure is conserved in different gram-positive bacteria. The overall structure of SrtB in S. aureus as shown in the figure, consists of a unique eight-stranded β-barrel core structure and a two-helix subdomain at the N-terminal end. SrtB is similar in structure to SrtA with rmsd of 1.25Å but SrtB has more peripheral helices It has an N-terminal helical bundle and an α-helix between β6 and β7. The N-terminal extension present in SrtB relative to SrtA is very significant. It is known to place the two termini on the same side of the protein. This is believed to result in a different orientation of the protein on the surface of the cell, potentially affecting substrate access.

Native GLP-1 is degraded within minutes by circulating enzymes. The synthetic version carries substitutions at positions that block enzymatic cleavage, plus a fatty acid side chain that promotes albumin binding. These two changes together extend circulation time from minutes to roughly a week.
Albumin binding keeps a large fraction of the compound in a slowly released reservoir within the bloodstream. Plasma levels decline gradually rather than falling sharply after each administration. That profile supports dosing intervals measured in days instead of hours.
The active peptide sequence is the same in both formats. The oral version adds an absorption enhancer that is not present in the injected solution. Differences in excipients and formulation affect uptake rather than the identity of the active molecule.
It is a synthetic analog of GLP-1 produced through medicinal chemistry to resist enzymatic degradation. The design goal was longer circulation than the native hormone.
