Receptor activation follows the canonical Gs pathway: binding increases intracellular cyclic AMP, which promotes protein kinase A activity. In pancreatic beta cells this amplifies glucose-dependent insulin release, so secretion rises when blood glucose is high and changes little when it is low. The same signalling suppresses glucagon release from alpha cells and slows gastric emptying. Receptors in the hypothalamus and brainstem are thought to contribute to reduced appetite and lower energy intake. Which of these effects dominates clinical outcomes remains an area of active study.
Semaglutide is a synthetic peptide analogue of glucagon-like peptide-1, a gut hormone released by intestinal L cells after food intake. The natural hormone acts on pancreatic and central receptors but is degraded within minutes by dipeptidyl peptidase-4 and other peptidases. Semaglutide belongs to the class of long-acting GLP-1 receptor agonists, a group distinguished by structural changes that slow breakdown and extend circulation time. Its development followed earlier short-acting analogues and reflects a general strategy in peptide drug design: preserve receptor activity while blocking proteolytic clearance.
Serum protein binding dominates the pharmacokinetic profile. The attached chain associates strongly with albumin, shielding the peptide from enzymatic attack and slowing filtration by the kidney. This interaction extends the circulation half-life to roughly one week in humans, which supports weekly administration intervals. An oral version pairs the peptide with an absorption enhancer that transiently alters gastric epithelium, permitting limited uptake; bioavailability by that route is substantially lower than by injection.
Semaglutide belongs to the glucagon-like peptide-1 receptor agonist class, a group of synthetic peptides that imitate an incretin hormone released by intestinal L cells after food intake. Native GLP-1 circulates for only a few minutes because dipeptidyl peptidase-4 cleaves it rapidly. The hormone acts on pancreatic islets, the gastrointestinal tract, and several brain regions. Because the natural peptide is short-lived, development work concentrated on analogues that keep receptor activity while resisting enzymatic breakdown and renal clearance.

Stability studies examine how temperature, pH, and moisture influence degradation rates over time. In aqueous solution, hydrolysis and deamidation accelerate as pH moves away from mildly acidic conditions. Light exposure and residual metal ions can also trigger oxidation of susceptible residues. Accelerated aging at elevated temperature is used to estimate shelf life, though extrapolation to room temperature carries uncertainty because individual degradation pathways do not always scale predictably.
Lyophilized semaglutide powder is typically held at minus twenty degrees Celsius for long-term storage. At that temperature, solid-state degradation reactions proceed slowly and the peptide remains intact for extended periods. Repeated freeze-thaw cycles are best avoided because they promote aggregation and can shift the proportion of monomeric peptide present. Working aliquots are often prepared so that each portion is thawed only once, and desiccant is placed inside the vial to limit moisture uptake.
Identity and purity are usually assessed by reverse-phase high-performance liquid chromatography coupled to mass spectrometry. Retention time and observed mass are compared against a reference standard run under identical conditions. Impurity profiles reveal deamidation products, oxidized methionine variants, and truncated fragments that arise during synthesis or storage. Peptide mapping through enzymatic digestion confirms the primary sequence, while amino acid analysis offers an independent check on overall composition.
The distinction between established facts and open questions matters here. That the peptide binds the GLP-1 receptor and stimulates insulin release in a glucose-dependent manner is well documented. How individual variability in receptor density, gastric emptying rate, and gut microbiome composition shapes response remains an active research area. Long-term outcomes beyond five years of continuous use are not yet fully characterized in published trials, and several extension studies are ongoing.
Semaglutide is a synthetic peptide analog of glucagon-like peptide-1, a hormone released from intestinal L cells after food intake. The molecule is a 31-amino-acid backbone modified at three positions to resist cleavage by dipeptidyl peptidase-4, the enzyme that degrades native GLP-1 within minutes. A lysine residue at position 26 carries a linker and a C18 fatty diacid, which promotes binding to serum albumin and slows renal clearance. These changes extend the circulating half-life from roughly two minutes to about one week in humans.
The sequence incorporates alpha-aminoisobutyric acid at position 8, replacing the alanine found in the natural hormone. This substitution blocks the primary DPP-4 recognition site and contributes most of the enzymatic stability. Albumin binding further protects the peptide and reduces the frequency of administration required to maintain active plasma levels. Because the fatty acid chain increases lipophilicity, the compound is formulated as a solution rather than a simple aqueous buffer. Researchers describe the design as an incremental optimization of earlier GLP-1 analogs rather than a wholly new scaffold.
The company that developed the compound filed it as a long-acting analogue, and it gained first approval in 2017 for type 2 diabetes. Later authorisations from several regulators extended the indication to chronic weight management, and the World Health Organization added the glucagon-like peptide-1 receptor agonist drug class to its model list of essential medicines in 2023. Production uses solid-phase peptide synthesis followed by side-chain conjugation and chromatographic purification. Supply constraints and cost differences across regions are well documented. Literature on long-term outcomes continues to grow, with many trials reporting surrogate endpoints rather than hard clinical endpoints.
Semaglutide is a synthetic peptide of thirty-one amino acids that shares roughly ninety-four percent sequence identity with human glucagon-like peptide-1. Two substitutions resist enzymatic cleavage by dipeptidyl peptidase-4, and a fatty diacid side chain attached through a linker promotes binding to serum albumin. That albumin binding slows renal clearance and extends the circulating half-life from minutes to approximately one week. The structural changes are well established in the published literature. Whether the same modifications affect receptor signalling bias in ways that matter clinically remains an open question.
Pharmacological activity arises from agonism at the glucagon-like peptide-1 receptor, a G protein-coupled receptor expressed in the pancreas, the gastrointestinal tract, and the brainstem. Receptor activation raises intracellular cyclic adenosine monophosphate and enhances insulin release in a glucose-dependent manner, an effect that diminishes when blood glucose concentration is low. Other effects include slowed gastric emptying and hypothalamic satiety signalling. These pathways are described well. Receptor desensitisation rates across tissues, relative to the endogenous hormone, are still under investigation, and reported findings differ between laboratories.

The growing complexity of surface-bound molecular systems has driven the development of increasingly refined surface analysis methods. In parallel, advances in high-performance computing have enabled wider use of computational modeling and simulation in the investigation of molecular-scale systems.
In 2004, GSK agreed to settle charges of consumer fraud for $2.5 million. The legal discovery process also uncovered evidence of deliberate, systematic suppression of unfavorable Paxil research results. One of GSK's internal documents read, "It would be commercially unacceptable to include a statement that efficacy [in children] had not been demonstrated, as this would undermine the profile of paroxetine". In 2012, the United States Department of Justice fined GlaxoSmithKline $3 billion for withholding data, unlawfully promoting use in those under 18, and preparing an article that misleadingly reported the effects of paroxetine in adolescents with depression following its clinical trial study 329. In February 2016, the UK Competition and Markets Authority imposed record fines of £45 million on companies that were found to have infringed European Union and UK Competition law by entering into agreements to delay the market entry of generic versions of the drug in the UK. GlaxoSmithKline received the bulk of the fines, being fined £37,600,757. Other companies that produce generics were issued fines which collectively total £7,384,146. UK public health services are likely to claim damages for being overcharged in the period where the generic versions of the drug were illegally blocked from the market, as the generics are over 70% less expensive. GlaxoSmithKline may also face actions from other generic manufacturers who incurred losses as a result of the anticompetitive conduct.
==== Binge eating disorder ==== Binge eating disorder (BED) is characterized by recurrent and persistent episodes of compulsive binge eating. These episodes are often accompanied by marked distress and a feeling of loss of control over eating. The pathophysiology of BED is not fully understood, but it is believed to involve dysfunctional dopaminergic reward circuitry along the cortico-striatal-thalamic-cortical loop. As of July 2024, lisdexamfetamine is the only USFDA- and TGA-approved pharmacotherapy for BED. Evidence suggests that lisdexamfetamine's treatment efficacy in BED is underpinned at least in part by a psychopathological overlap between BED and ADHD, with the latter conceptualized as a cognitive control disorder that also benefits from treatment with lisdexamfetamine. Lisdexamfetamine's therapeutic effects for BED primarily involve direct action in the central nervous system after conversion to its pharmacologically active metabolite, dextroamphetamine. Dextroamphetamine increases dopaminergic and noradrenergic neurotransmission in prefrontal cortex, a region of the brain that regulates cognitive control of behavior. Similar to its therapeutic effect in ADHD, dextroamphetamine enhances cognitive control and may reduce impulsivity in patients with BED by enhancing the cognitive processes responsible for overriding prepotent feeding responses that precede binge eating episodes.
==== Francesco Giorgi ==== Francesco Giorgi is the former parliamentary assistant and domestic partner of Eva Kaili. He was arrested in December 2022 before being released with an electronic tag in February 2023 having agreed a deal with the prosecutor to reveal details of the illegal organisation. According to his LinkedIn profile, Giorgi studied political science at the University of Milan. Giorgi started his relationship with Kaili in 2017. At the time Giorgi worked as an assistant at the European Parliament to Antonio Panzeri. Giorgi and Kaili have a child together. Giorgi was arrested by Belgian authorities mid-December 2022 in the context of the Qatargate scandal. He admitted having accepted bribes and alleged his partner Kaili was not directly involved in the corruption scheme. Giorgi is alleged to have helped channel funds from Qatar and Morocco to influence European institutions through setting up the association Fight Impunity together with Antonio Panzeri. In February 2023 Giorgi was released from prison with an electronic tag. Italian prosecutors have initiated an investigation in March 2023 into activities of two unnamed suspects who would have acted as collaborators of Antonio Panzeri and Giorgi. The Italian investigation related to a company called Equality Consultancy in Milan set up by a former accountant of Antonio Panzeri and Giorgi's father Luciano and brother Stefano in 2018. The brother and father of Giorgi left the company after a year and are not being investigated by Italian authorities.

=== Dextran hydrogel colon-targeting === The colon is an ideal target for dextran hydrogel drug delivery systems due to the presence of dextranases. Dextran can be cross-linked with diisocyanate to form a hydrogel that can be loaded with hydrocortisone to treat swelling or inflammation in the colon. Hydrogels can also be synthesized from crosslinking epichlorohydrin (ECH) with dextran. Dextran-ECH hydrogels can be loaded with salmon calcitonin (sCT) to treat bone diseases. Dextran-ECH hydrogels loaded with sCT achieved comparable release rates to other polymeric hydrogels in the colon.
== Pathology == It has been suggested that these cell rests are the histogenetic origins of Brenner tumors, due to the histological similarity of the epithelium of Walthard cell rests and Brenner tumors to the urothelium of the lower urinary tract. Also, it has been proposed that Brenner tumors and Walthard cell rests signify urothelial differentiation within the female genital tract.
When exposed to low oxygen concentrations, haemoglobin S polymerises into long strands within red blood cells (RBCs). These strands distort the shape of the cell and, after a few seconds, cause it to adopt an abnormal, inflexible, sickle-like shape. This process reverses when oxygen concentration is raised, and the cells resume their normal biconcave disc shape. If sickling takes place in the venous system, after blood has passed through the capillaries, it does not affect the organs, and the RBCs can unsickle when they become oxygenated in the lungs. Repeated switching between sickle and normal shapes damages the membrane of the RBC so that it eventually becomes permanently sickled. Normal red blood cells are quite elastic and have a biconcave disc shape, which allows the cells to deform to pass through capillaries. In sickle cell disease, low oxygen tension promotes red blood cell sickling and repeated sickling episodes damage the cell membrane and decrease the cell's elasticity. These cells fail to return to normal shape when oxygen tension is restored. As a consequence, these rigid blood cells are unable to deform as they pass through narrow capillaries, leading to vessel occlusion and ischaemia. Sickled cells are detected as they pass through the spleen and are destroyed. In young children with sickle cell disease, the accumulation of sickled cells in the spleen can result in splenic sequestration crisis. In this, the spleen becomes engorged with blood, depriving the general circulation of blood cells and leading to severe anaemia.

Both Neanderthals and modern humans took care of the elderly members of their societies during the Middle and Upper Paleolithic. Some sources claim that most Middle and Upper Paleolithic societies were possibly fundamentally egalitarian and may have rarely or never engaged in organized violence between groups (i.e. war). Some Upper Paleolithic societies in resource-rich environments (such as societies in Sungir, in what is now Russia) may have had more complex and hierarchical organization (such as tribes with a pronounced hierarchy and a somewhat formal division of labor) and may have engaged in endemic warfare. Some argue that there was no formal leadership during the Middle and Upper Paleolithic. Like contemporary egalitarian hunter-gatherers such as the Mbuti pygmies, societies may have made decisions by communal consensus decision making rather than by appointing permanent rulers such as chiefs and monarchs. Nor was there a formal division of labor during the Paleolithic. Each member of the group was skilled at all tasks essential to survival, regardless of individual abilities. Theories to explain the apparent egalitarianism have arisen, notably the Marxist concept of primitive communism. Christopher Boehm (1999) has hypothesized that egalitarianism may have evolved in Paleolithic societies because of a need to distribute resources such as food and meat equally to avoid famine and ensure a stable food supply. Unresolved complex mixture (UCM), or hump, is a feature frequently observed in gas chromatographic (GC) data of crude oils and extracts from organisms exposed to oil. The reason for the UCM hump appearance is that GC cannot resolve and identify a significant part of the hydrocarbons in crude oils. The resolved components appear as peaks while the UCM appears as a large background/platform. In non-biodegraded oils the UCM may comprise less than 50% of the total area of the chromatogram, while in biodegraded oils this figure can rise to over 90%. UCMs are also observed in certain refined fractions such as lubricating oils and references therein. In attempting to determine "the processes that regulate the fate of petroleum following release to the environment,” geochemist Christopher M. Reddy of Woods Hole Oceanographic Institution invented an application of comprehensive two-dimensional gas chromatography (GCxGC) that resolves UMPs and that he patented. As it degrades in a marine environment, oil undergoes complex transformations, producing residues composed of extremely complex organic mixtures that accumulate in such “protective environments” as fiddler crabs and marsh grass. These residues form the majority of the unresolved complex mixture (UCM) resulting from the breakdown of crude oils that GC had previously been unable to resolve but which Reddy’s novel GCxGC application has made accessible, enabling determination of “the underlying processes controlling petroleum fate” as it degrades in a marine environment.
The Canadas were the main front in the War of 1812 between the United States and the United Kingdom. Peace came in 1815; no boundaries were changed. Immigration resumed at a higher level, with over 960,000 arrivals from Britain between 1815 and 1850. New arrivals included refugees escaping the Great Irish Famine as well as Gaelic-speaking Scots displaced by the Highland Clearances. Infectious diseases killed between 25 and 33 percent of Europeans who immigrated to Canada before 1891. The desire for responsible government resulted in the abortive Rebellions of 1837. The Durham Report subsequently recommended responsible government and the assimilation of French Canadians into English culture. The Act of Union 1840 merged the Canadas into a united Province of Canada and responsible government was established for all provinces of British North America east of Lake Superior by 1855. The signing of the Oregon Treaty by Britain and the United States in 1846 ended the Oregon boundary dispute, extending the border westward along the 49th parallel. This paved the way for British colonies on Vancouver Island (1849) and in British Columbia (1858). The Anglo-Russian Treaty of Saint Petersburg (1825) established the border along the Pacific coast, but, even after the US Alaska Purchase of 1867, disputes continued about the exact demarcation of the Alaska–Yukon and Alaska–British Columbia border.
Hallucinogens can be divided into three broad categories: psychedelics, dissociatives, and deliriants. They can cause subjective changes in perception, thought, emotion and consciousness. Unlike other psychoactive drugs such as stimulants and opioids, hallucinogens do not merely amplify familiar states of mind but also induce experiences that differ from those of ordinary consciousness, often compared to non-ordinary forms of consciousness such as trance, meditation, conversion experiences, and dreams. Psychedelics, dissociatives, and deliriants have a long worldwide history of use within medicinal and religious traditions. They are used in shamanic forms of ritual healing and divination, in initiation rites, and in the religious rituals of syncretistic movements such as União do Vegetal, Santo Daime, Temple of the True Inner Light, and the Native American Church. When used in religious practice, psychedelic drugs, as well as other substances like tobacco, are referred to as entheogens. Hallucinogen-induced psychotic disorder occurs when psychosis persists despite no longer being intoxicated with the drug. It is estimated that 26% of people with hallucinogen-induced psychotic disorder will transition to a diagnosis of schizophrenia. This percentage is less than the psychosis transition rate for cannabis (34%) but higher than that of amphetamines (22%). Starting in the mid-20th century, psychedelic drugs have been the object of extensive attention in the Western world.
=== NMR spectroscopy === Hydrogen and deuterium nuclei are grossly different in their magnetic properties. Thus it is possible to distinguish between them by NMR spectroscopy. Deuterons will not be observed in a 1H NMR spectrum and conversely, protons will not be observed in a 2H NMR spectrum. Where small signals are observed in a 1H NMR spectrum of a highly deuterated sample, these are referred to as residual signals. They can be used to calculate the level of deuteration in a molecule. Analogous signals are not observed in 2H NMR spectra because of the low sensitivity of this technique compared to the 1H analysis. Deuterons typically exhibit very similar chemical shifts to their analogous protons. Analysis via 13C NMR spectroscopy is also possible: the different spin values of hydrogen (1/2) and deuterium (1) gives rise to different splitting multiplicities. NMR spectroscopy can be used to determine site-specific deuteration of molecules. Another method uses HSQC spectra. Typically HSQC spectra are recorded at a series of timepoints while the hydrogen is exchanging with the deuterium. Since the HSQC experiment is specific for hydrogen, the signal will decay exponentially as the hydrogen exchanges. It is then possible to fit an exponential function to the data, and obtain the exchange constant. This method gives residue-specific information for all the residues in the protein simultaneously The major drawback is that it requires a prior assignment of the spectrum for the protein in question.

Native GLP-1 is a short-lived peptide cleared within one to two minutes by dipeptidyl peptidase-4 and related enzymes. Semaglutide keeps the receptor-binding backbone but adds substitutions and a lipid chain. These changes block the main cleavage site and allow reversible albumin binding, extending the half-life to roughly 165 hours.
Albumin is the most abundant protein in plasma and carries molecules that bear fatty-acid chains. Binding shields the peptide from renal filtration and from peptidases, keeping a circulating reservoir. Slow release from this reservoir produces sustained receptor occupancy and supports infrequent dosing.
The insulinotropic effect is glucose-dependent, meaning secretion increases mainly when glucose is elevated. This property is often described as lowering the chance of hypoglycaemia when the compound is used alone. Other glucose-lowering agents used at the same time can still cause low blood glucose.
It is a synthetic peptide of 31 amino acids, built to resemble the natural incretin hormone GLP-1. Because of its size and composition it is handled analytically like other therapeutic peptides, using chromatographic and mass spectrometric methods rather than the techniques typical of small organic drugs.
